to be honest I don't think such a thing exist, I think it's going to have to be a DIY job... If you do go that route please do a little journal on it, you will bound to be help someone else out with a similar problem.

It's not answering your question, but I did make a custom panel to fit above the top sash of my double hung window, and used an exhaust fitting from Amazon.
